Getting Started
Welmivia Medical is a concierge psychiatric practice for cognitive and mental-health optimization, serving patients across Virginia and Florida via telehealth. All care is led by Dr. Sonia Mia Diaz, M.D.
We combine evidence-based psychiatric medication management with precision tools — including pharmacogenomic testing (BrightKaire), GLP-1 and metabolic health management, and lab-driven evaluations — delivered through a membership model built around depth and direct access.
We operate entirely outside of insurance, by design. That means no prior authorizations, no time limits, and a physician who actually knows your case.
Welmivia is designed for adults who want precise, proactive psychiatric care — not reactive, insurance-paced management. You may be a good fit if you:

Welmivia is not a crisis service and does not provide emergency mental health care. We serve established adult outpatients seeking high-quality ongoing management.
There are two ways in:
- Free 10-Min Fit Call — A brief screening call to confirm that Welmivia is the right fit before you commit. No clinical advice is given on the call.
- Precision Psychiatric Evaluation — Book directly if you're ready. This is a comprehensive 90-minute visit with Dr. Diaz ($450), required for all new patients. You'll leave with a personalized Welmivia Blueprint — your treatment roadmap.
Welmivia serves patients statewide across Virginia and Florida via HIPAA-compliant telehealth — no in-office visits required. You can be anywhere in either state.
You must be physically located in Virginia or Florida at the time of each visit. We cannot see patients located in other states.
Pricing & Insurance
No. Welmivia is a direct-pay practice — we do not bill any insurance company, including Medicare or Medicaid.
This is intentional. Insurance-based psychiatry introduces authorization delays, visit-length restrictions, and financial incentives that work against thorough, personalized care. Operating outside of that system lets us practice medicine the way it should work.
Out-of-network reimbursement: We provide itemized superbills upon request. Many patients with PPO plans or HSA/FSA accounts submit these and recover a meaningful portion of their visit costs. Individual clinical visits (not membership fees) are submittable with standard CPT codes.
The Precision Psychiatric Evaluation is $450. It's a 90-minute comprehensive visit with Dr. Diaz covering diagnostic assessment, symptom and medication history, risk review, lab ordering, and personalized Blueprint delivery.
If you'd like pharmacogenomic testing (BrightKaire) ordered at the time of your evaluation: the ordering fee is waived if Dr. Diaz recommends it. If you request it and are not yet a member, a $50 ordering fee is added. Members have BrightKaire ordering included.
After your initial evaluation, you can continue as an à la carte patient with per-visit billing:
- 15-minute follow-up: $150
- 30-minute follow-up: $250
- Annual 60-Min Precision Health Review: $300
À la carte patients do not have access to BrightKaire ordering at member rates, monthly check-ins, secure messaging priority, or GLP-1/metabolic management. Membership is required for those services. (BrightKaire ordering carries a $50 fee for non-members who request it; the fee is waived if Dr. Diaz recommends it.)
Individual clinical visit fees (evaluations, follow-ups, and the Precision Health Review) are generally HSA/FSA-eligible. We recommend confirming with your plan administrator, as eligibility rules vary by account type.
Membership fees may or may not be eligible depending on your specific HSA/FSA plan. We are happy to provide documentation to support your submission — consult your plan for guidance.

Annual memberships save approximately two months of billing — $3,250/yr for Core and $5,250/yr for Signature.
No. After your initial evaluation, you can continue as an à la carte patient, scheduling follow-up visits individually as needed.
However, membership is required if you want access to: ongoing GLP-1 or peptide management, BrightKaire ordering at no charge, monthly check-ins, prioritized secure messaging, or the annual Precision Health Review at no additional cost. These services are reserved for members.
Membership enrollment happens after your Precision Psychiatric Evaluation and Blueprint. The steps are simple:
- Choose your tier — Core or Signature — and billing cadence (monthly or annual)
- An enrollment invoice is generated through your patient portal
- Recurring billing is established securely via our Bluefin payment system
- Your included visits and services activate immediately for the membership period
Annual memberships are strongly recommended — they eliminate monthly admin friction and save approximately two months of fees.
Yes. Members can add on visits beyond their monthly inclusion at a discounted member rate:
- Core members: 15-min add-on $100 · 30-min add-on $200
- Signature members: 15-min add-on $75 · 30-min add-on $150
These add-on visits are available to members only and are not available to à la carte patients.
Clinical Care & Services
Welmivia provides psychiatric medication management and cognitive optimization for adults. Primary areas include:

Welmivia is an outpatient optimization practice — not an acute care or crisis service. If you are in immediate danger, please contact 988 (Suicide & Crisis Lifeline) or go to your nearest emergency room.
BrightKaire (by NeuroKaire) is a pharmacogenomic blood test that uses AI analysis of patient-derived neurons to identify which antidepressants are most likely to be effective for you — removing much of the trial-and-error typically involved in antidepressant selection.
The test is billed directly by NeuroKaire and is covered under Medicare Part B and many select private insurance plans. Results take approximately 8 weeks. Dr. Diaz reviews results with you at a follow-up visit and adjusts your plan accordingly.
For members (Core and Signature): BrightKaire ordering is included — no additional charge.
For non-members, physician-recommended: The ordering fee is waived.
For non-members, patient-requested: A $50 ordering fee applies.
Yes — for members. GLP-1, peptide, and metabolic management is offered as a monitored adjunct to psychiatric care within your scheduled membership visit time. It is not a standalone service.
Welmivia manages titration, lab monitoring, and clinical oversight. Medications are always billed separately — patients source them directly from vetted pharmacies and pay for them independently. Welmivia never bills for medications.
Active membership is required for ongoing monitored access to these therapies.
Dr. Diaz orders labs as clinically appropriate — routine panels typically include CBC, CMP, TSH, and targeted add-ons based on your treatment. For GLP-1 and metabolic management, additional metabolic and safety labs are ordered as indicated.
Labs are billed directly by LabCorp or Quest — not by Welmivia. Lab ordering and interpretation is included for members. Results are reviewed with you at your next follow-up visit.
Many patients find that lab costs are covered or partially offset by their insurance even when Welmivia is out-of-network, since LabCorp and Quest bill independently.
Welmivia is a psychiatric medication management practice. Supportive psychotherapy is integrated naturally within scheduled clinical visit time — particularly during follow-ups and the annual Precision Health Review.
Welmivia does not offer standalone therapy sessions, and open-ended messaging is not used as a clinical tool. If structured psychotherapy (CBT, DBT, EMDR, etc.) is part of your care plan, Dr. Diaz can provide a referral recommendation alongside your medication management.
The Annual Precision Health Review is a comprehensive 60-minute deep-dive available to all patients. For members, it's included. For à la carte patients, it's available at $300.
The review covers: psychiatric status and medication efficacy, metabolic markers and lab trends, cognitive optimization goals, progress against your Blueprint, and care plan for the year ahead. Think of it as your annual clinical reset — broader in scope than a standard follow-up.
Telehealth & Logistics
All Welmivia visits are conducted via HIPAA-compliant video through the patient portal. You'll receive a link before each appointment — no downloads or accounts beyond the patient portal are required.
You can join from a computer, tablet, or smartphone. You must be physically located in Virginia at the time of each visit.
Secure messaging is available to all patients through the patient portal and is intended for scheduling coordination, brief clinical updates, and question routing — not ongoing clinical consultations or therapy.
- À la carte patients: Standard response window (48–72 business hours)
- Core members: Staff-triaged with a faster response target
- Signature members: Direct physician response — your message goes to Dr. Diaz, not staff
- New patients: Book a Fit Call or Precision Evaluation directly from the Welmivia website
- Established patients (à la carte): Schedule follow-ups through the patient portal
- Members: Access internal member scheduling through the portal for your included and add-on visits
Medical cannabis evaluations are scheduled through a separate, dedicated booking page — fully independent of the main Welmivia practice flow.
Welmivia is a legitimate psychiatric practice operating within all applicable Virginia and Florida medical board and DEA regulations. Controlled substance prescribing (including ADHD medications) follows standard clinical protocols and is governed by applicable state and federal telehealth prescribing rules.
All monitored therapies require an established patient relationship, ongoing clinical oversight, and — for GLP-1 and metabolic therapies — active membership.
Billing & Superbills
A superbill is an itemized receipt that includes everything your insurance company needs to process an out-of-network reimbursement claim: CPT codes, diagnosis codes, provider NPI, service date, and fees paid.
Welmivia provides superbills upon request for all clinical visits. You submit the superbill directly to your insurance company — Welmivia does not file claims on your behalf.
Reimbursement rates vary by plan. Patients with PPO plans or robust out-of-network benefits often recover a significant portion of visit costs. Membership fees are generally not reimbursable, but individual visit fees are submittable.
Welmivia never bills for medications. All prescriptions — antidepressants, stimulants, GLP-1s, peptides, and any other medications — are filled at your pharmacy of choice (standard retail or vetted compounding pharmacy) and billed directly to you by the pharmacy.
Welmivia's fee covers clinical evaluation, monitoring, titration, and lab oversight. The medication itself is always a separate transaction.
Some administrative services carry fees for all patients, with discounts applied for members:
- Letter of Medical Necessity (ADHD accommodations, disability, insurance appeals): Standard $125 · Members $75

Signature members have after-hours/urgent access and prior authorization support included with no additional fee.
