Please review the following before booking or using our services.

Appointment Cancellations
We require at least 24 hours' notice to cancel or reschedule an appointment. Cancellations made with less than 24 hours' notice will be charged the full visit fee. No-shows are charged the full visit fee.

Membership Fees

Monthly membership fees are non-refundable once the billing cycle has begun. Annual memberships may be prorated at the physician's discretion under extenuating circumstances. Contact us directly to discuss your situation.

Initial Evaluation ($450)

The initial evaluation fee is fully refundable if cancelled at least 24 hours in advance. Cancellations within 24 hours of the scheduled evaluation are non-refundable.

How to Cancel

Please cancel through the patient portal or by contacting us at admin@welmivia.com or (434) 623-0003. We cannot accept cancellations via social media or the general contact form.

By accessing Welmivia Medical's website and services, you agree to these terms. They apply to all patients, visitors, and users of welmiviamedical.com.

Direct-Pay Model

Welmivia Medical is a direct-pay concierge psychiatric practice. We do not bill insurance directly. Superbills are provided upon request for potential out-of-network reimbursement, but we cannot guarantee coverage or reimbursement from your insurance plan.

Scope of Services

Welmivia provides telehealth psychiatric evaluations, medication management, and related services to residents of Virginia and Florida. Services are physician-led and require a completed initial evaluation before ongoing care is established.

Telehealth Agreement

By scheduling a telehealth appointment, you consent to receiving care via secure video or phone as permitted under applicable law. You must be physically located in Virginia or Florida at the time of your appointment.

Not for Emergencies

Welmivia is not an emergency service. If you are experiencing a mental health crisis, please call or text 988 (Suicide & Crisis Lifeline), call 911, or go to your nearest emergency room.

These terms are subject to change. Continued use of our services constitutes acceptance of updated terms.

Information We Collect

We collect information you provide directly, including name, contact details, health history, and payment information necessary to provide care. We also collect limited technical data through our website to improve functionality.

How We Use Your Information

Your information is used to provide and coordinate your care, process payments, communicate about appointments, and comply with legal requirements. We do not sell or share your personal information with third parties for marketing purposes.

HIPAA

Protected health information (PHI) is handled in compliance with HIPAA. You have the right to access, amend, and request restrictions on the use of your PHI. Please contact us at admin@welmivia.com to exercise these rights.

Data Security

We use industry-standard security measures including encrypted communications and our HIPAA-compliant EHR platform (CharmHealth) to protect your information.

No Physician-Patient Relationship

Viewing or using this website does not establish a physician-patient relationship. A relationship is established only when you complete an initial evaluation with Dr. Diaz and are accepted as a patient of Welmivia Medical.

Welmivia Medical provides telehealth services to patients located in the Commonwealth of Virginia and the State of Florida. By scheduling and attending a telehealth appointment, you acknowledge the following:

  • You must be physically located in Virginia or Florida at the time of your appointment.
  • Telehealth services are delivered via secure, HIPAA-compliant video or phone platforms.
  • Telehealth has known limitations compared to in-person care and may not be appropriate for all conditions.
  • Dr. Diaz holds a Virginia medical license (License No. 0101288149) and a Florida Telehealth registration for Medical Doctor (THLT License No. 8518).
  • You have the right to refuse telehealth services and receive referrals to in-person care.
  • Technical difficulties may occasionally interrupt sessions; we will reschedule promptly at no additional charge.
